Labcorp's Patient Rights and Obligation Form for Genetic Counseling
This Rights and Obligations form covers the Genetic Results Counseling you scheduled with a Labcorp genetic counselor.
In addition to providing genetic counseling services, Labcorp also performs genetic testing.
During genetic results counseling, a genetic counselor will do the following as applicable:
- Review Your Genetic Test Results. Discuss the individual genetic test finding(s), based on your specific genetic results (or your child’s) and any inherited genetic conditions identified by the testing completed.
- Discuss Risk and Family Implications. Explain your (or your child’s) risk based on these genetic findings, explain how the finding(s) may be inherited, and the possible impact of the results on family members based on the completed test results.
- Follow up on Surveillance and Screening and Additional Testing. Direct you to speak with your (or your child’s) healthcare provider about available health management along with any follow-up genetic testing that may be indicated for you based on your test results and/or personal and family history.
If you have additional questions or concerns about your family history or medical management beyond the scope of the results being discussed, we recommend consulting your healthcare provider, who may refer you for a comprehensive genetic counseling session.
Important responsibilities and notes:
- Accuracy of Information: The genetic counselor will only use the information you provided and your test results. Please ensure all information shared with the genetic counselor is as accurate and complete as possible. If you learn any new or updated information related to the genetic issue or results discussed, it is your responsibility to contact the genetic counselor following your appointment.
- Follow-Up Testing: If you decide to have any follow-up genetic testing that you discussed with the Labcorp genetic counselor, it is your responsibility to ensure the testing is performed. Your healthcare provider is responsible for ordering testing, selecting the appropriate laboratory, and providing the results.
- Recording Policy: Our policy prohibits audio or video recording of the Genetic Results Counseling session.
